Adult Practice Reviews
In accordance with the Social Services and Well Being (Wales) Act 2014, the Regional Safeguarding Board must arrange for an Adult Practice Review to be held where abuse or neglect of an adult is known or suspected and the adult has:
- died
- sustained potentially life threatening injury or
- sustained serious and permanent impairment of health
The purpose of the review will be to identify if there is any learning for future practice and to promote future improvements in relation to inter-agency adult protection practice. A Learning Event will be held which will be attended by professionals who had involvement with the adult and/or family. An information leaflet (78kb PDF) has been produced for professionals which will explain the purpose of the Learning Event and what can be expected from those who attend.
Professionals who wish to refer a case for consideration of an Adult Practice Review are asked to complete the APR/CPR Practice Referral Form (20kb DOC) and return to the Safeguarding Boards’ Business Unit cardiffandvalersb@cardiff.gov.uk

Thematic Review into Adult Deaths: A research project was commissioned by the National Independent Safeguarding Board (NISB) to look at adult deaths in Wales from an analysis of reviews, including Domestic Homicide Reviews, Adult Practice Reviews and Mental Health Homicide Reviews.
